Home Health Services

At Haven Memory Care convenience is a focus for our residents as well as their loved ones. As a result, we work hard to make sure all needs are met within our community. Home health services such as physical, occupational and speech therapy occur in our community. Imaging, heart monitoring and scans are also done at our community with mobile units. There’s no need to worry about traveling to appointments all over the city.  All the local hospice companies are welcome in our community. We believe competition raises the bar of service. While we do partner with a local hospice company, residents and their families are free to select whichever hospice company they prefer. The hospice companies are not associated with Haven Memory Care or our employees.

Hospice Care Services

Transitional Care Physicians of America is a local company, with a national sounding name, that we have partnered with. This group provides customer service above any other group we have found. They respond within 15 minutes of our requests for service. They are in our community weekly without request however they will come every day if needed. They have a geriatric psychiatric team which is unique to our area and very important. There are a lot of complications with this disease and the very nature of this disease is the brain. Having this service available is not only extremely necessary but helpful as well. It takes another trip out of the community away from the residents’ routine. They also have a portal which can be accessed by any authorized family member. All lab reports can be reviewed remotely by family. The family can communicate directly with the team through this portal as well.

Pharmacy

At Haven Memory Care we have also partnered with a local pharmacy that has been in Athens for generations. We us Adds Pharmacy however there is a unique facet to this partnership and pharmacy. They have established a separate location that services the Athens area’s senior living communities. They have deliveries to our community twice a day but are willing to come at a moment’s notice in case of urgent medication needs. If your loved one goes to the hospital and returns to the community with a different pharmaceutical need, Adds Pharmacy can accommodate these needs without interruption. Like Haven Memory Care they are locally owned and operated. We support local businesses whenever we can to keep our community partnered with the Athens Community.  If you decide to use a different pharmacy company you are free to do so however all medications must be repackaged by Adds Pharmacy as required by state law.

  • My mom has Alzheimer’s and it sucks, it is heartbreaking and frustrating and so hard when you realize that you are no longer able to provide the care she needs. This is the situation my dad and I found ourselves in and we were lost as to what was best. We tried an assisted living facility that had a memory care unit, and it was awful. She was not receiving the care she needed, the staff was not properly trained and it was an overall bad experience. Thank God we found Haven. It is locally owned and operated, it only does memory care. All the staff are not only trained properly, they are the kindest most patient people I have ever met. They love my mother, and they take such good care of her. It is intimate location, all the residence are well loved and it is such a warm and inviting atmosphere. My mom feels at home and my dad and I finally have peace of mind that she is in the right place. Lisa, the owner is a true Saint. Her mom passed away from dementia so she just gets it. I have cried in her office on more than one occasion. I can’t express how truly grateful I am that we found Haven. It has been a life changer.

Our Memory Care Community

Surrounded by 11 acres of hardwoods and the natural beauty, our private pay memory care community integrates design elements that are purposefully crafted to support the unique needs of those with dementia. Our secure community is architecturally designed by a specialized memory care architect to be easy to navigate and features calm, soothing colors and plenty of natural light. Each element contributes to creating a serene, comfortable environment that makes our residents feel at home.

Garden Area

Our garden is enclosed and available for the residents to enjoy at will. It features a porch with ceiling fans, garden lights, sun sails for shade, a water feature, and sensory garden. We have recently sodded the area with barefoot soft grass. It is also a haven for birds as residents may have bird feeders outside their windows. Currently, there are black-capped chickadees, tufted titmouse, cardinals, morning dove, and house wren just to mention a few, and don’t forget the hummingbirds when in season. We also pride ourselves on the view of the backyard where we host about 20 deer. Staff feed the deer corn and apples and have a salt lick for them so they’re fun to watch. There’s plenty of seating room and a paved walking path in the courtyard.

It takes into account the resident’s medical history, cognitive abilities, physical limitations, behavioral challenges, and personal preferences. The goal of the care plan is to enhance the quality of life and maximize independence while addressing any potential safety concerns or medical needs.

Care Plan Details

At Haven Memory Care, our care plans are based on person-centered care. Person-centered care, previously “patient-centered care,” focuses on the individual needs of patients while considering their environment, abilities, feelings, experiences, and wants.

Of course, this type of care is not just limited to patients in acute care but rather includes residents in our community as well. When we take this approach to care we are able to optimize the resident experience by providing them and family members with ample opportunity to assist in making medical choices. Under person-centered care, all clinical decisions are made with empathy and deep respect for resident values, preferences, and rights. 

Put simply, person-centered services are about taking the time to get to know your residents. “It is only by knowing a person that you can figure out what matters to them,” said Dr. Mike Wasserman, former President of the California Association of Long Term Care Medicine. After 30 days, we reevaluate the care plan because so much can happen in the first 30 days. Once the custom care plan is in place it is revisited every quarter to change as the needs of our residents change.

What Are ADLs?

ADL stands for Activities of Daily Living. All of our resident’s ADLs are taken care of in one way or another. If they are unable to complete these tasks we will either complete them or we will assist based on their level of need. We will always take care of laundry and housekeeping. But for other ADLs such as hygiene, toileting, grooming, and eating, we take more of an assistance approach based on the level of progression of their diagnosis.
